Анотація
Вступ. Лікування пацієнтів із переломами середньої зони обличчя становить значну частину щоденної практики в щелепно-лицевій хірургії, а самі травми є наслідком насильницьких злочинів, спортивного травматизму, побутових нещасних випадків та дорожньо-транспортних пригод. Проте воєнний час трансформує систему надання травматологічної допомоги, вимагаючи нової парадигми лікування та реабілітації пацієнтів з унікальною та комплексною анатомією ушкоджень, спричинених летальною зброєю.
Мета. Дослідити епідеміологію, підходи до лікування та клінічні характеристики пацієнтів із переломами середньої зони обличчя до та після повномасштабного вторгнення в Україну на базі однієї цивільної лікарні.
Матеріали та методи. Проведено ретроспективне когортне дослідження за участю 364 пацієнтів. Для порівняння характеристик пацієнтів когорту було розділено на дві групи залежно від часу госпіталізації: до повномасштабного вторгнення (включені з 2017 року до 23 лютого 2022 року) та після нього (особи, які проходили лікування починаючи з 24 лютого 2022 року). Дані були отримані з медичних карт, включаючи демографічні показники, етіологію та тип травми, супутні ушкодження, вид проведеного лікування та час, що минув від моменту травми до оперативного втручання.
Результати. Ми спостерігали зменшення кількості кримінальних травм після початку повномасштабного вторгнення (49,0% до та 23,08% після відповідно). До 2022 року ізольовані переломи вилицевого комплексу становили переважну більшість усіх діагнозів (70,44%), тоді як після 24 лютого 2022 року ці ушкодження становили лише 30,76%. Значно зросла кількість пацієнтів з ізольованими переломами орбіти (10,93% у період до повномасштабного вторгнення та 34,18 % після початку). Відкрита репозиція та внутрішня фіксація була найпоширенішим методом лікування в обох групах (87,85% та 86,32% відповідно). Оскільки кількість пацієнтів із бойовою травмою різко зросла, ми також зафіксували збільшення кількості випадків багатоуламкових переломів та ушкоджень із дефектами кісткової тканини.
Висновки. Початок активних бойових дій змінив структуру травм, що спостерігаються в цивільних лікарнях. Відкрита репозиція та внутрішня фіксація залишаються основними методом лікування. Тяжкість бойових травм розширює сферу застосування пацієнтспецифічних імплантатів.
